We are seeking a compassionate and dedicated Community Worker to join our team.
In this role, you will provide support and assistance to individuals in their homes and the community, helping them to achieve their goals and maintain their independence.
Key Responsibilities:
- Provide personal care and support to clients, including assistance with daily living activities.
- Assist clients with mobility and transportation to appointments and social activities.
- Support clients in developing and maintaining social connections.
- Implement care plans and follow instructions from healthcare professionals.
- Maintain accurate records and documentation of care provided.
- Communicate effectively with clients, their families, and other team members.
Requirements:
- Relevant qualifications in community services, aged care, or disability support.
- Previous experience in a similar role is preferred.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver’s license and reliable transportation.
- Current First Aid and CPR certification.
- National Police Clearance and Working with Children Check (or willingness to obtain).
